Registration checklist

_____ 1. COPY OF A BIRTH CERTIFICATE OR PASSPORT:
The Club keeps this copy. California Youth Soccer Association will not accept a hospital certificate. All players new to Montclair cannot be registered without this. Please return by mail.

Note: Returning recreational players U15 and below, and returning Clipper players do not need to supply this. Players playing in the U16 group recreational teams for the first time do need to provide this.

Clippers teams turn this into their team registration helper.
 
__________2. 1" BY 1" PHOTO OF YOUR PLAYER (FOR U9-U19). ALL COACHES AND ASSISTANT COACHES MUST PROVIDE A PHOTO. U6 and U8 age groups do not need a photo.

We prefer photos to be uploaded into Sportability. If you are unable to upload, you may mail in.

The photo must be full face, no profile, no sunglasses, no hats, eyes must be open. Write your child's name on the back of the photo and either use clear tape or a paperclip to attach it to a full size sheet of paper so that it does not get lost. No staples please. PLEASE do not photocopy a passport photo to use for your player pass photo, they just don't turn out clear enough. Photos are very important, please take a little time to find one of good quality that fits on the player pass.

PLEASE TRIM YOUR PHOTO. It must be able to fit on a player pass the size of a drivers license.

DO NOT WEAR hats, baseball caps, sunglasses or glasses that turn into sunglasses. The eyes must be seen.

_____3. COACH DISCLOSURE FORM 1628 for head coaches and assistant coaches. If you are playing in CYSA. If you are a competitive NorCal team please contact the registrar.

You will receive a copy via email. Beginning in 2011, electronic signatures are acceptable. You no longer need to turn in this form if you register online. Please remember that coaches in U9-U19 still need a photo turned in. Please no hats or sunglasses.

_____4. JLYSSL Zero Tolerance Policy Form :
All parents and coaches are required to sign this document in order to play on a Montclair Soccer team or within Jack London Soccer League. You will turn this into your team PCA coordinator. 

_____6. JLYSSL Coach Agreement Form:
This is only required for Coaches and Assistant Coaches. .You will turn this form into your team PCA coordinator.

Montclair Soccer Club Financial Aid Program

MSC is committed to working with families to be sure every child can enjoy a soccer experience with our club. With this commitment we have developed various levels of programs with the intention that families can find one that best fits their financial abilities and child’s ambitions. We strive to have each of our programs meet or exceed the comparable standards for similar programs found in the Bay Area.

To help families budget for their specific soccer program with MSC we offer the flexibility of using an installment payment plan. By going through the regular registration process you can set up this plan to pay in up to 8 equal installments. This can minimize the financial impact of having to pay for a full program up-front.

After assessing these options within our Club, a family may still need some financial help to have their child participate. For these families we do want to be sure that your child can find an opportunity to gain a soccer experience. MSC will gladly review your specific situation to determine the level of financial support we can offer each year. The level of support varies each year depending the Club’s overall financial abilities and the number of families requesting support. Our first priority is to be sure that as many kids as possible will be able to participate in one of our programs.

To help us assess the amount of financial support we can offer your family we ask that you please fill out the Montclair Soccer Club Financial Aid Application and email it back to our Club Financial Aid Officer at financialaid@montclairsoccer.org. We try to ask the very minimum information that will help us most fairly distribute our current funds. This information is kept with the highest level of confidentiality possible with a very limited number of Club Officials reviewing each application. Generally we only need our Financial Aid Officer reviewing each application. We anticipate that the 2012 assistance will be at the following levels:
>> Recreation Program: $100
>> Clippers Programs: $250

Higher Awards may be determined after reviewing overall budget and family needs.
